Procedures and Forms

The St. Brendan application process consists of five steps:

1. Consider reserving a spot for a tour of our campus. Call our office at 415-731-2665 between 9:00 am and 3:00 pm starting on September 11, 2023 to secure your reservation. You will find the dates on the Tour Dates tab to the right. 
 
2. To the right is a link to the application packet.  For kindergarten, please submit the following by December 8, 2023. (Applications for siblings of current students are due November 21, 2023.)  Applications for grades 1-6 may be submitted at any time.
 
  • The completed Application Form;
  • A recent photograph of child with parents;
  • $60.00 application fee (non-refundable, payable to St. Brendan School);
  • The completed Date and Time Preference Form for individual child assessment and parents' interview (Kindergarten only);
  • A copy of the child’s birth certificate; 
  • If Catholic, a copy of your child's baptismal certificate;
  • A copy of the applicant’s most recent report card and standardized test scores (grades 1-6 only);
 
3. For kindergarten, download and print the Confidential Preschool Recommendation Form and give it to your child’s preschool teacher. The teacher will submit the recommendation to St. Brendan School directly. Please request that the form be submitted no later than December 8, 2023.

4. Schedule an Individual Child Assessment and Parents' Interview.
  • Appointments may be scheduled on one of the following days:                                                                                       December 11, 12, and 18, 2023                                                                                                                                January 9 and 10,  2024 
  • Please submit the Date and Time Preference Form. Please provide as many choices as you can. Priority will be given to those who submit their forms early.
  • During the screening, parents will meet with our principal, Ms. Lakatta.
  • Assessments and interviews are held in the St. Brendan Gymnasium Conference Room 201 Ulloa Street, and will take a total of approximately 25 minutes.

5. The children will attend a group evaluation in the kindergarten classroom on Saturday, January 6, 2024.
