Procedures and Forms

The St. Brendan application process consists of five steps:

1. Consider reserving a spot for a tour of our campus. Call our admissions office at 415-731-2770 between 9:00 am and 3:00 pm starting September 2026, to secure your reservation. You will find the dates on the Tour Dates tab to the right. 
 
2. To the right is a link to the application packet.   Applications may be submitted at any time.
 
  • The completed Application Form;
  • $60.00 application fee (non-refundable, payable to St. Brendan School);
  • A copy of the child’s birth certificate; 
  • If Catholic, a copy of your child's baptismal certificate;
  • A copy of the applicant’s most recent report card and standardized test scores (grades 1-6 only);
  • If applicable, Medical or Psycho-Educational Evaluation Reports from private testing or from SFUSD or any other school district (including any IEP paperwork), resulting in a diagnosis of any learning differences such as ADHD, Dyslexia, Dysgraphia, etc. 
 
3. For kindergarten, download and print the Confidential Student Recommendation Form for Kindergarten and give it to your child’s preschool teacher. The teacher will submit the recommendation to St. Brendan School directly. 

4. Submit the application materials to 940 Laguna Honda Blvd, San Francisco, CA 94127.
