Required Forms for Start of School:
Forms #1-8 are due by the first day of school, Tuesday, August 20. Please note whether each form requires one per child or one per family. Attach all of the forms to one email for your family and send it to [email protected].
For your convenience, all of the files are writeable PDFs. You can download these forms and open them using Adobe Acrobat. Then, you can save them to your computer, fill them in electronically, sign them, and send them to [email protected]. They no longer need to be printed.
